The church was built outside the Aurelian Walls on the site where, according to tradition, the relics of the apostles Peter and Paolo had been transferred in 258 to save them from the persecutions; they were later returned to their original places of burial once the churches dedicated to them had been built.
The martyr, a Roman soldier, victim of the persecutions of the third century, is buried in the catacombs beneath the church. Here, in the early fourth century, the Emperor Constantine built a large basilica, initially dedicated to the memory of the Apostles and later dedicated to San Sebastiano. Destroyed by the Saracens and rebuilt in the ninth century, the current form of the basilica dates from reconstruction work commissioned in the seventeenth century by Cardinal Scipione Borghese.

Along the Via Appia, in a natural depression in the ground that housed a pagan necropolis, a Christian cemetery developed from the mid-third century AD. From 258 AD, as witnessed by the creation of a courtyard and a sacred shrine, liturgies were held in honour of the apostles Peter and Paul; the invocations of the faithful scratched on the walls can still be seen. At the remembrance of Peter and Paul, at slightly later time, the burial of the body of St. Sebastian, martyred during the persecutions of the third century, is recalled. Above the place of worship, Constantine (306-337) built a basilica dedicated to the Apostles, which later became the Basilica of San Sebastiano fuori le mura.
